Request Name Change

This service is used to request a name change.

Who can access this service?

Anyone with a Motlow account

Why access this service?

Your account name is incorrect or your name has changed.

How do I access this service?

Students

Click Here and navigate to Student > Admissions and Records > Change of Record and complete the form. Admissions will submit a ticket once the request has been approved. 

Faculty

Click Here and navigate to Employee > Human Resource Forms > Change of Record and complete the form. HR will submit a ticket to IT once the request has been approved.

Must be Motlow faculty, staff or student
